We work with all insurance companies and Medicare. We will need all your demographic and insurance information prior to your appointment. On the day of your appointment, you will need to bring your insurance card, photo id and any other forms that will assist in making sure your claim is filed correctly.

At the time of service, you will be responsible for all fees that are not covered by your insurance, including copays, coinsurance, deductibles, and non-covered services such as disability and FMLA forms. For your convenience, we accept cash and all major credit cards.

These fee are not eligible for reimbursement from medical insurance or health savings plans.
